VOLUNTARY LIFE & AD&D

Plan Rules:

  • Must be actively at work on effective date

  • employee must elect coverage for themselves to elect coverage for dependents

  • benefit amount doubles in the event of an accident which results in a dismemberment or death

  • if enrolling for the first time, health questions are required.

What is Life & AD&D Insurance?

Voluntary plans allow the employee to decide whether or not they want to enroll in life insurance and AD&D. The employee typically covers the premium costs for any voluntary coverage they elect. For voluntary plans, employers can typically determine a range of
coverage amounts for their employees to choose from.
